The authors searched CENTRAL, MEDLINE, SCOPUS, Google Scholar, the WHO trials portal, ClinicalTrials.gov, the Clinical Trial Registry of India and the AYUSH Research Portal for randomised controlled trials comparing ashwagandha extract with placebo in adults aged 18 and over[1].

Five trials with 400 participants met the criteria. Pooled, ashwagandha showed a small but significant effect on overall sleep, with a standardised mean difference of −0.59 (95% CI −0.75 to −0.42) and moderate heterogeneity between studies (I² = 62%)[1].

Three conditions made the effect more prominent[1]:

  • participants had a diagnosis of insomnia, rather than being healthy sleepers;
  • the dose was at least 600 mg per day;
  • the trial ran for at least eight weeks.

The review also reported improvements in mental alertness on rising and in anxiety levels, no significant effect on quality of life, and no serious side effects across the included trials[1].

The dose and duration thresholds, visualised

Those three conditions are the practical heart of the finding. A 300 mg dose taken for two weeks is not the intervention that produced the pooled effect.

What the individual trials measured

Beneath the pooled figure, the individual studies are informative because they used objective sleep parameters rather than only questionnaires.

In a double-blind, randomised, placebo-controlled trial in insomnia and anxiety, sleep onset latency — the time taken to fall asleep — was significantly shorter after 10 weeks on ashwagandha root extract than on placebo (p = 0.019). Sleep efficiency rose from 75.63 to 83.48 in the ashwagandha group, against 75.14 to 79.68 on placebo[2].

A separate parallel-group study compared healthy volunteers with insomnia patients across sleep onset latency, total sleep time, wake after sleep onset, time in bed and sleep efficiency. Both groups improved, but the improvement was more marked in the insomnia group, with sleep onset latency and sleep efficiency the most improved parameters (both p < 0.0001)[3].

The pattern is consistent: the more disturbed the baseline sleep, the more room there was to move.

Honest limitations

Five trials and 400 participants is a small evidence base for a herb sold at this scale. The heterogeneity figure of 62% indicates the trials did not all point the same way to the same degree[1]. A more recent systematic review of Withania somnifera for anxiety and insomnia reached a similar conclusion — promising results, but larger samples are needed before the effects can be considered settled[4].

There is also a regulatory limitation that no amount of research changes. Ashwagandha has no authorised health claim on the GB Nutrition and Health Claims Register, so a UK food supplement cannot state that it improves sleep, however the studies read. Where UK sleep formulas do make a permitted claim, it comes from a nutrient such as magnesium, which contributes to normal psychological function — not from the botanical.
